Added decoding of To:, Cc:, Resent-To:, Resent-Cc:, and
Resent-From: addresses to mhl.format and mhl.headers.  Thanks to
Conrad Hughes for suggesting it for the first two components and
to Ralph for suggesting it for the Resent- components.
